Admissions at Voorhees Pediatric Facility

At VPF, we strive for a streamlined and transparent admissions process, prioritizing both the medical and emotional needs of your child. Here’s how our referral and admissions process works:

  • Primary Referrals: While the majority of our referrals come from New Jersey, we welcome and accept referrals from other states and countries.
  • Initial Evaluation: Upon receiving a referral, our medical team conducts an on-site evaluation either at the child’s home or the referring hospital. This assessment determines the child’s skilled nursing needs.
  • Family Consultation: We highly encourage families to tour our facility and meet with our medical team. This is an opportunity to address any questions or concerns about the placement of your child.
  • Waiting Lists: We maintain two separate waiting lists: one for non-ventilator dependent children and another for ventilator-dependent children.
  • Bed Allocation: When a bed becomes available, it is offered to the first child on the appropriate waiting list. Our medical team then confirms whether the child is medically stable for transfer to VPF.
  • Financial Approval: Before confirming the admission, we secure financial coverage, obtaining approvals from commercial payers or managed Medicaid providers as necessary.
  • Pre-Admission Tour: If not done prior, families are invited to tour VPF and meet our team within 7 days before the actual admission.
  • Medical Documentation: Our admissions team obtains all necessary pre-admission testing and medical documentation within the 7 days leading up to the admission.
  • Family Communication: We place a high value on open communication between our clinical team and the families we serve.

Submit Initial Documentation

To expedite your child's admission into our specialized pediatric care program at VPF, initial documentation is required. These documents provide us with the essential information needed to create a personalized care plan that addresses the unique needs of your child.

Processing of Referral

At Voorhees Pediatric Facility, a small team will review the medical appropriateness for your child’s admissions to our facility.

Insurance Verification

If your child is medically appropriate to be admitted to VPF, you insurance eligibility will be verified by our admissions team.

On-site Evaluation/Small Team Review

Once all prevalent clinical information is received, a liaison will go out to complete an on-site evaluation to determine medical appropriateness for Voorhees Pediatric Facility.

Once on-site evaluation is complete the liaisons will present their findings to the Admissions Team to assess all clinical information and determine if referral is appropriate for our facility.

If your child is not medically appropriate for VPF, a denial letter with suggestions and recommendations of other appropriate pediatric facilities will be sent to the family.

Family Meeting/Transition of Care Meeting

It’s normal to have tons of questions about our pediatric facility and the services we provide. That’s why a family meeting is scheduled with the small VPF team to go over any questions or concerns you may have.

A Transition of care of meeting will be held prior to admission, allowing our team and your child's current medical team to review their care plan and ensure continuity of care.

Day of Admission

On the day prior to admissions, the physician of the referring hospital must call to sign off to the VPF medical team. While on the day of admission, the VPF nursing staff calls the referring hospital to receive nurse sign off. Additionally, on the day of, there’s transportation assistance and complimentary meal service for your child and family.

On the day of admissions you must bring the following documentation:

  • Birth certificate
  • Social security card
  • Insurance card
  • Clothing

Post-Admission

After your child’s successful admission to VPF, our focus shifts to providing personalized and comprehensive care tailored to your child’s medical and emotional needs.

Care Team Meeting

Within 21 days of admission, our VPF social worker coordinates a care team meeting that includes an interdisciplinary team of specialists. Families are not just welcome but are encouraged to actively participate in setting up the plan of care for their child.

Recognizing that families may face various challenges such as demanding schedules or geographic distance, phone conference calls are an option for participation. This meeting is crucial in establishing open lines of communication and ensuring that your child receives the highest quality of pediatric care.

Designated Social Worker

Each child is assigned a designated social worker, serving as your liaison for clinical questions and medical issues. They play an essential role in the healthcare team, ensuring that the family stays well-informed and involved in the child’s care.

Dedicated Primary Nurse

A primary nurse will be specially assigned to each child. This nurse takes the lead in administering your child’s daily nursing needs and becomes the go-to medical professional for your child during their stay.

Primary Respiratory Therapist

For children requiring respiratory care, a primary respiratory therapist will be assigned. This specialized therapist works closely with the medical team to manage and optimize your child’s respiratory health.

At VPF, we also offer in-house physician and nurse practitioner services, ensuring that specialized medical care is readily available when needed.
